We make every effort to assist you with your insurance. We do require your complete dental insurance information at least 2 business days prior to your child’s appointment, so that one of our staff can obtain detailed information for you and get verification of your dental coverage. Once this is obtained, we will offer the courtesy of filing most major dental insurance for you, but contact our staff for questions about specific plans. We are not contracted with any dental insurance plan and are normally considered out of network which may result in higher out of pocket co-insurance and lower dental benefits. Any amount not covered by your dental plan, deductibles, or co-insurance will be collected at the time of service. You will be responsible for any additional balances after your insurance payment is received.
In this ever-changing economy, we realize that many families are no longer able to afford dental insurance with sufficient coverage to make a difference. For this reason, our practice will now offer a 10% discount off our fees for any family that does not have dental coverage (and for which we are not filing an insurance claim).
We do our best to properly estimate your portion at each visit and we collect this portion at the time of service. Occasionally, there may be a balance on your account after the insurance company pays their portion. We will send you a statement for this unpaid balance after we receive the insurance payment and we are certain that the balance is owed by you. In the event that the insurance company is not sending payment in a timely manner (within 60 days of the appointment), we will request payment from the parent or guardian and the eventual insurance benefit will be forwarded to them.
The parent or guardian that brings the child for the initial visit is ultimately responsible for the final payment of services. We are not able to intervene in the matters of divorce.
We accept cash, checks, MasterCard, Visa, and Discover as payment. We also use a third party finance company called CareCredit that has extended payment plans for dental and medical services. You can apply for and secure a credit line now by going to www.carecredit.com or obtain a form in our office.
We are also a specialist provider for Ameriplan. Ameriplan allows a discount percentage of our fees for active Ameriplan members, but it cannot be combined or used if we are filing insurance. Go to www.ameriplanusa.com for information on membership.
For our patients 18 years and older
If someone other than the parent or legal guardian will be bringing the patient to an appointment, please download and email or fax our Authorization for Minor Child form. The parent or guardian must bring the patient to their initial or first appointment in our office.
Moving? Please complete the Transfer of Records form and email or fax to us, so that we can forward your child's information.
